Nightmare Mode in Resident Evil Requiem introduces new challenges and significantly more enemies. We’ve summarized all the key differences and changes.
The Nightmare Mode in Resident Evil Requiem is not just a simple increase to health and damage values. Instead, the mode deliberately changes enemy placements, item locations and certain gameplay mechanics. While the overall structure of the chapters remains the same, many familiar situations play out differently and force you to adapt your usual strategies. Here are the most important changes at a glance.
More enemies at key locations
One of the most noticeable changes is the increased enemy density in certain areas. However, the game does not simply add more enemies everywhere. Instead, it reinforces specific encounters.
In the sanatorium you will encounter additional zombies in early rooms. Areas that were often just quick stopovers during a normal playthrough become significantly more dangerous.
For example:
- An additional zombie appears in the lounge and can catch you off guard while crossing the room.
- Several zombies wait inside the pantry, turning what was previously a quick detour into a chaotic fight.
The difficulty also increases later in the game. Stronger enemies such as super mutants appear more frequently, especially in narrow corridors or laboratory areas, forcing you to move much more carefully.

Changed item distribution in the sanatorium
In Nightmare Mode, important items are sometimes placed in different locations. This means you can no longer rely on your memory from the first playthrough.
Some of the most noticeable changes include:
- The pantry key is no longer found near the cook but instead located in another room of the building.
- Antique coins are no longer placed at their usual locations. Instead, they can appear on defeated zombies.
- The large, bulky monster in the sanatorium drops several coins at once.
The use of coins also changes. Instead of the familiar upgrades from the standard mode, you gain access to different options, such as additional ammunition or a special knife.
Adjustments to certain mechanics
In addition to enemies and items, Nightmare Mode also alters certain gameplay mechanics. In the basement of the sanatorium, the shadow monster can still be driven away with the Requiem, but it now requires significantly more hits. While a single shot is often enough in normal mode, several hits are needed here before the creature retreats.
A scene in the garage area also plays out differently:
- The excavator does not stop immediately.
- You must also shoot the engine to fully stop the machine.
These changes may seem small at first but significantly alter the timing of many encounters.
New threats in familiar scenes
Several familiar scenes become more difficult in Nightmare Mode. In the church, additional enemies appear after the first attack wave. Just as you start to stabilize the situation, more zombies and sometimes stronger enemies enter the fight.
Other scenes also become more dangerous:
- In the tunnel near the garden house, an active super mutant waits during the escape.
- In front of the church you encounter significantly more zombies than in the standard playthrough.
- In Raccoon City there are more enemies roaming the streets much earlier in the game.
Open areas that previously felt relatively calm now become far more dangerous. Use cars or surrounding obstacles to move through these sections safely.

Closed shortcuts and longer routes
Especially later in the game, Nightmare Mode also changes certain routes. A convenient shortcut in the laboratory area, for example, remains locked. As a result, you must take a longer path through several dangerous rooms.
Other changes affect individual rooms:
- More zombies appear simultaneously in the clean room.
- In some bioweapon storage areas you encounter multiple lickers in tight spaces.
Boss fights and stronger enemies
The major boss fights themselves change very little in terms of mechanics. However, enemies can withstand more damage, which makes these encounters last longer. The key difference often lies in your preparation. Because you need more resources for combat in Nightmare Mode, you often reach boss fights with less ammunition and fewer healing items.
Powerful regular enemies also appear more frequently, especially in tight laboratory sections. Multiple lickers or additional elite enemies can make previously familiar rooms far more dangerous. Nightmare Mode therefore changes the bosses themselves only slightly, but ensures that you reach them in a much worse state of preparation.
